Dear Sir/Madam: 

Please provide this office with all documentation regarding the lost wages for the employee referenced above.  Please be sure and include Mr. Loyd's earning amount, any sick leave he might have lost and any overtimes he may have lost.  Please also be sure and include any "donated time" given to Mr. Loyd by other employees.  The lost wages occurred from the date of the accident of January 2, 2018 through March 8, 2018.

Please also be sure and provide the contact information for the person providing the documents requested above.
